Caring for Older Adults with Cancer: Who, When, Where, Why, and How? - T. Jolly - 20170712

Age is the single most important risk factor for developing cancer and the incidence of most human cancers increases with advancing age. Older adults frequently have co-morbidities, functional and psychosocial deficits which may impact cancer care. This lecture discuss the effect of aging on patients with cancer and reviews of the use of simple geriatric assessment tools in oncology. Case studies are shared to illustrate how such tools can be used to guide medical decision making for these patients who often have co-morbidities and functional and/or psychosocial deficits. The session traces the historical evolution of prognostic disclosure in pediatrics—from a "never tell" default through the mid-20th century to the "always tell" recommendation that emerged by the late 1980s. The lecture highlights key foundational and contemporary studies on prognostic disclosure. A central theme is that prognostic communication is not a single event but an evolving, relational process. A patient's story is used to anchor this principle by illustrating shifting patient engagement, parents acting as filters of medical information, the "next step" framing of difficult conversations, and the tension between "knowing everything" and "knowing enough."
